Employee killed when struck by falling I-beam

At approximately 3:00 p.m. on September 24, 1992, Employee #1 was working with a crew that was lowering a 6 in. wide and 12 ft long I-beam from a level approximately 25 ft to 30 ft above his level. The 144 lb I-beam had been used to support a conveyor. An air trigger along with a 3/8 in. diameter wire rope cable and several snatch blocks were attached to the I-beam. There was some slack in the cable where Employee #1 was standing and he signaled the air trigger operator. It is believed that as they were trying to take up the slack in the line, the beam fell approximately 25 ft to 30 ft. It fell through a 4 ft by 4 ft opening in the intermediate floor, striking Employee #1 in the head. Although he was wearing a hard hat, he suffered fatal injuries. One beam had been removed prior to the accident.
