Employee dies when struck in head by falling conveyor

On May 6, 1991, Employee #1 and three coworkers were setting up a concrete pipe form to begin a run of arched pipe. The conveyor used to carry concrete to the form had been raised to its highest level to give the employees room to work. They had some difficulty in getting the header, which formed the female end of the pipe, to seat properly in the form. They lifted the header out of the jacket, or pipe form, with a hoist, to maneuver it into position. As the header was being lifted, it swung enough to slightly jar the conveyor, which had a single pipe support. The pipe support consisted of an outer, upper T resting on a length of rebar inserted through holes in the lower sleeve. When the conveyor was jarred, the upper sleeve was lifted enough to disengage from the lower one. The pipe support collapsed and the conveyor fell, striking Employee #1 on the head. He died as a result of injuries sustained in the accident.
