Investigation Summary
At approximately 8:15 p.m. on February 23, 1992, Employee #1 was operating a JLG manlift (serial #0300014688) with 60 ft extension boom at the McDermott, Inc., fabrication yard in Port of Iberia, LA. He was welding electrical tray hangers on an off-shore platform under construction. He was attempting to maneuver the basket on the end of the extension boom, trying to reach an electrical cable tray hanger on the rig approximately 40 ft in the air. When he found that he could not reach the hanger, he tried to maneuver the basket out of the relatively small area. He became lodged between the structural I-beam on the outer edge of the platform and the back part of the basket. One of the controls was jammed by the boom. Employee #1 suffered a crushing injury to the chest. He died. There were no witnesses to the accident. Employee #1 may have had a misperception of his environment in relation to the task at hand.
