Investigation Summary
At 6:30 a.m. on April 19, 1993, Employee #1 arrived at the construction site, asking for work. He signed papers and started work at 7:00 a.m. He was cautioned not to step on the skylights, and not to step on any yellow-surfaced unsupported insulation. He was given instruction on climbing the ladder 30 ft to the rooftop. He and four other laborers were placing a metal roof panel weighing 95 to 100 lb and measuring 3 ft by 30 ft. They stood on the panels already in place to toss this panel onto the insulation in front of them. Employee #1 was at one end, and he started to take a step forward when he realized he was about to step on a skylight that everyone had repeatedly admonished him to avoid. He jumped over it and landed on the insulation, which gave way. At 9:07 a.m., Employee #1 died of the injuries sustained in the fall.
