Investigation Summary
ON SEPTEMBER 12, 1989, AN OILFIELD CREW WAS ASSIGNED TO REMOVE AND REPLACE A BROKEN SUBMERSIBLE PUMP AT JP#1 OIL WELL. THE HORSEHEAD ON THE LUFKIN PUMPING UNIT, M-228D-213-86, SERIES J, WAS DISMANTLED, THE RODS WERE PULLED OUT, AND THE PUMP WAS REPLACED. THE HORSEHEAD WAS RE-INSTALLED WITHOUT BEING SECURELY BOLTED TO THE WALKING BEAM. AT APPROXIMATELY 2:50 PM, EMPLOYEE #1 WAS ASSISTING IN THE INSTALLATION OF THE BRIDLE ON THE ROD. A HOIST WAS USED TO RAISE THE POLISH ROD UPWARD TO MAKE AN ADJUSTMENT FOR THE HANGER. AS THE ROD MOVED UPWARD, THE BOX ON THE POLISH ROD CAUGHT ON THE BOTTOM OF THE UNSECURED HORSEHEAD AND FLIPPED IT OVER AND TO THE SIDE. THE UNBOLTED AND UNLATCHED 570-POUND HORSEHEAD FELL TO THE GROUND AND STRUCK EMPLOYEE #1 IN THE CHEST, FATALLY INJURING HIM.
