Investigation Summary
At 3:00 p.m. on January 19, 2015, Employee #1, employed by a plastering company, was working at a multifamily residential building construction project. He was standing on a scaffold and applying plaster by hand to pop-outs. A plank on the scaffold platform broke, and Employee #1 fell to the ground, a fall height of 5 feet.
