WHILE EMPLOYEE #1 WAS WORKING UNDER AN AUTOMOBILE ON A HOIST, THE HOIST FELL APPROXIMATELY TWO FEET. THE EMPLOYEE RECEIVED INJURIES TO THE HEAD AND SHOULDERS FROM THE FALLING HOIST AND AUTOMOBILE.
