Investigation Summary
At approximately 6:35 p.m. on January 23, 2015, an employee was switching one of the line carts carrying a trailer due to a malfunction on the wheels. The damaged line cart needed to be replaced with a functional line cart. The back of the trailer was lifted by a single powered industrial truck to remove the damaged line cart while the front end of the trailer remained on a functional line cart. Once elevated, the trailer rolled ahead and slid off the forks of the powered industrial truck. The employee was underneath the trailer at the time the trailer slid and was crushed and killed by the trailer. The employee died from multiple blunt force injuries.
