Employee injured when elevated work cage falls

Employee #1 was in a duct shaft installing duct from a work cage that was raised and lowered by an electric hoist. He incorrectly hooked the hoist to the work cage. The hook came loose, allowing him to fall. He sustained multiple bruises and contusions to his arms, but did not require hospitalization.
