THUMB & FINGERS CAUGHT IN DIE OF FOOT-POWERED PRESS

EMPLOYEE #1 WAS RUNNING A FOOT-POWERED PRESS, ASSEMBLING OIL PUMPS, WHEN SHE PLACED HER THUMB BETWEEN THE DIE AND THE PART. SHE KICKED THE PRESS, DISLOCATING THE KNUCKLE ON HER RIGHT THUMB. EMPLOYEE #2 WAS RUNNING THE SAME TYPE OF PRESS, PRESSING PARTS FOR ASSEMBLY. A PART BECAME STUCK IN THE DIE. AS SHE WAS TRYING TO REMOVE THE STUCK PART, SHE INADVERTENTLY KICKED THE PRESS, SMASHING THREE FINGERS BETWEEN THE DIE AND THE PART.
