CONSTRUCTION EMPLOYEE KILLED IN FALL FROM UNGUARDED SCAFFOLD

AT 11 AM ON APRIL 2, 1987, EMPLOYEE #1 CLIMBED A LADDER TO THE TOP OF A 16 FOOT WALL FORM AND OVER ONTO A SCAFFOLD ON THE OPPOSITE SIDE OF THE WALL. HE WAS CARRYING TWO HAND TROWELS AND A BRUSH TO PLACE ON THE SCAFFOLD. THERE WAS NO PERIMETER GUARDING OR OTHER FALL PROTECTION PROVIDED ON THE SCAFFOLD. EMPLOYEE #1 FELL 13 FEET, 10 1/2 INCHES TO THE GROUND, SUFFERING A FATAL SKULL FRACTURE AND HEAD INJURY.
