EMPLOYEE KILLED IN FALL OVER ROOF EDGE

AT APPROXIMATELY 11:25 AM ON MARCH 5, 1987, EMPLOYEE #1 WAS REMOVING GUARDRAILS AND CLEANING UP THE AREA WITH THE HELP OF A COWORKER. THE MASONRY CREW HAD JUST COMPLETED THEIR JOB OF POURING THE BOND BEAM AT THE TOP OF A 25 FOOT HIGH WALL FOR AN ELEVATOR SHAFT. EMPLOYEE #1 AND HIS CO-WORKER SET 14 OF 30 ANCHOR BOLTS IN THE BOND BEAM WHEN EMPLOYEE #1 FELL 25 FEET OVER THE EDGE OF THE ROOF, STRIKING HIS HEAD AGAINST THE AXLE OF A FORKLIFT PARKED BELOW. SCAFFOLDING WAS AVAILABLE FOR EMPLOYEE #1'S USE, AND IN FACT WAS USED BY THE MASONS FROM INSIDE THE SHAFT. A SEPARATE SCAFFOLD HAD BEEN ERECTED ALONG THE OUTSIDE WALL OF THE SHAFT AND COULD HAVE BEEN USED TO SET THE ANCHOR BOLTS.
