Investigation Summary
At 4:00 p.m. on January 12, 2015, an employee was repairing the transmission for a 2003 Chevrolet Silverado 2500 pickup truck. The employee did not set the trucks parking brake before lifting it into an unstable position. The truck rolled forward while the employee was underneath and it fell from the supports. A vehicle cross member fell directly onto the employees skull, crushing it and killing him.
