AS EMPLOYEE #1 WAS RELEASING THE REAR BINDING CHAIN ON A LOAD OF LOGS HE WAS CAUGHT BETWEEN THE FORKLIFT FINGER (LUMBER JACK) AND THE FRONT TIRE OF THE REAR TANDEM. HE WAS CRUSHED TO DEATH.
