FELL TO BOTTOM OF CHEMICAL REACTOR

EMPLOYEE #1 WAS ATTEMPTING TO REMOVE A METAL BEAM FROM THE LOWER PART OF THE INSIDE OF A CHEMICAL REACTOR. NO FALL PROTECTION OR LADDER WAS USED, AND FOOTING WAS VERY POOR. THE EMPLOYEE FELL 18 FEET TO THE BOTTOM OF THE REACTOR. THE VESSEL HAD BEEN PUMPED, AND SAMPLES TAKEN, PRIOR TO ENTRY. THE SECOND EMPLOYEE WAS ADMITTED TO THE HOSPITAL FOR EXHAUSTION.
