STRUCK BY FALLING PIECE OF PIPE

EMPLOYEE #1 WAS SECURING PIPES FOR CO-AX CABLE, WORKING INSIDE AT THE 700 FOOT LEVEL OF A RADIO TOWER. TWO OTHER EMPLOYEES WERE WORKING AT THE 800 FOOT LEVEL. A PIPE ATTACHED TO THE BLOCK ON THE LOAD LINE SLIPPED THROUGH THE ROPE HITCH AND FELL DOWN THE OUTSIDE OF THE TOWER. THE EMPLOYEES AT THE 800 FOOT LEVEL YELLED A WARNING TO THOSE BELOW THEM. EMPLOYEE #1 STUCK HIS HEAD OUTSIDE OF THE TOWER AND LOOKED UP TO SEE WHAT WAS FALLING. THE FALLING PIPE, 18 FEET LONG AND WEIGHING 60 TO 70 POUNDS, STRUCK AND KILLED HIM. HE WAS NOT WEARING A HARD HAT BUT WAS WEARING A SAFETY BELT AND LANYARD.
