EMPLOYEE #1, WHO WAS REPAIRING A VINEGAR VAT, WAS WORKING FROM AN UNGUARDED PLATFORM 10 FEET ABOVE FLOOR LEVEL. THE PLATFORM WAS 2 FEET WIDE AND 4 FEET LONG. HE SUDDENLY LOST HIS BALANCE AND FELL TO THE CONCRETE FLOOR. HE WAS KILLED.
