EMPLOYEE #1 ATTEMPTED TO CLIMB A STACK OF CARDBOARD BOXES CONTAINING METAL PARTS. AS HE WAS CLIMBING HE INADVERTENTLY PULLED THE TOP BOX OVER ONTO HIMSELF. THE FALLING BOX STRUCK HIM IN THE HEAD, CRUSHING HIS HEAD AND KILLING HIM.
