EMPLOYEE KILLED WHEN HEAD CAUGHT IN MIXER BLADES

EMPLOYEE #1 WAS CLEANING THE BOTTOM OF A LARGE MEAT MIXER. THE MACHINE HAD NOT BEEN LOCKED OUT, NOR WERE ANY SAFETY INTERLOCK DEVICES IN USE. THE GROEN MEAT KETTLE WAS EQUIPPED WITH AN AGITATOR THAT WAS RAISED PARTIALLY ABOVE THE KETTLE. AS EMPLOYEE #1 LEANED INTO THE KETTLE TO REACH THE BOTTOM ANOTHER EMPLOYEE INADVERTENTLY TURNED ON AN UNLABELED SWITCH THAT ACTIVATED THE MIXER. EMPLOYEE #1 WAS KILLED WHEN HIS HEAD WAS CAUGHT IN THE MIXER'S AGITATOR BLADES.
