Investigation Summary

Investigation Nr: 570127
Event: 04/15/1986
Ten employees overexposed to carbon monoxide

Lid and bowl printers had been moved to a new location over two to four weeks, installed, and put into operation. The printers use a propane burner to heat plastic lids or bowls before printing with UV cured inks. The ventilation system had not been reinstalled. Apparently, at the beginning of the third shift, midnight to 8:30 a.m., the air conditioning system was turned off because it was cold, and a Clark forklift B55244045 was in the area. Employees #1 through #10 started to experience headaches between 5:00 and 6:30 a.m. The employees met in the rest room and all were suffering from headaches, nausea, and dizziness. At 8:00 a.m., all employees except Employee #10 went outside. All ten employees, except Employees #2 and #3, who went to a family doctor, went to the hospital for blood tests. The employer was notified of the carbon monoxide overexposure and told other employees to report to hospitals.
