EMPLOYEE #1 WAS CHECKING WATER LEVELS IN BARGE BOTTOMS WHEN, FOR UNKNOWN REASONS, HE FELL INTO THE RIVER. ALTHOUGH LIFE VESTS WERE AVAILABLE, THE EMPLOYEE DID NOT WEAR ONE. HE DISAPPEARED INTO THE RIVER AND WAS FOUND THREE DAYS LATER.
