EMPLOYEE STRUCK AND KILLED BY PIECE OF FALLING PIPE

THREE EMPLOYEES WERE ASSIGNED TO REMOVE A LARGE OVERHEAD PIPE WHICH WAS NO LONGER NEEDED. THE EMPLOYES CUT OUT A SECTION OF PIPE, 10 FEET, 3 INCHES LONG, WEIGHING 348 POUNDS, AND ALLOWED IT TO FALL, UNCONTROLLED, TO THE FLOOR. THE PIPE STRUCK THE FLOOR, BOUNCED AND STRUCK EMPLOYEE #1, WHO WAS WORKING AT FLOOR LEVEL. HE WAS KILLED.
