Investigation Summary
Employee #1 and a coworker were placing bundles of roof decking sheets on trusses for the roof. The bundles were being lifted into place by a crane. As a bundle of decking was swung around into place, Employee #1 grabbed the cribbing on the end of the bundle. The cribbing came off and Employee #1 lost his balance, falling approximately 64 ft onto concrete. He was killed.
