Investigation Summary
ON OCTOBER 16, 1986, EMPLOYEE #1 WAS WORKING IN THE MILL SHOP ON SITE. AS HE WAS REACHING FOR A PIECE OF WOOD ON THE SHELF, ANOTHER PIECE FELL AND HIT HIM ON THE TEMPLE. HE DID NOT REPORT THE ACCIDENT TO THE PRIME CONTRACTOR. HE CLEANED HIMSELF UP (THERE WAS SOME BLOOD) AND CONTINUED TO WORK THE REST OF THE WEEK. ON SATURDAY HE WAS TAKEN TO THE HOSPITAL, WHERE HE DIED. AN AUTOPSY REVEALED THAT HE DIED OF A CEREBRAL HEMORRHAGE, PROBABLY CAUSED BY THE ACCIDENT WHICH HAD OCCURRED TWO DAYS EARLIER.
