CONSTRUCTION EMPLOYEE INJURED IN FALL FROM WORK PLATFORM

AT APPROXIMATELY 1:15 PM ON NOVEMBER 16, 1987, EMPLOYEE #1 WAS WORKING FROM A PLATFORM WHICH HAD BEEN LIFTED BY A FORKLIFT. AS ANOTHER EMPLOYEE WAS HANDING UP PLYWOOD, EMPLOYEE #1 MOVED TO ONE SIDE OF THE PLATFORM. THE PLATFORM, WHICH WAS NOT SECURED TO THE FORKLIFT'S FORKS, FELL OVER SIDEWAYS. THE EMPLOYEE FELL TO THE FLOOR AND WAS STRUCK IN THE HEAD BY THE PLATFORM WHICH FELL AFER HIM. EMPLOYEE #1 WAS HOSPITALIZED FOR HIS INJURIES.
