Investigation Summary
Employee #1 and a coworker had climbed 20 ft to the roof of a house to nail a piece of siding onto the house. Employee #1 stepped off the edge of the roof, which was 39 in. wide and being used as a work platform, and fell onto a concrete patio. He died from injuries to his body and head.
