Investigation Summary
Employee #1 was run over by the right rear of a truck and killed. The driver had not told him to check any mechanical items under the truck and he did not know why Employee #1 had been there. OSHA recommended that in the future the driver should check under and around the truck for employees who might be under the trailer. Employees should be warned to stay out from under parked vehicles that are subject to being removed unexpectedly at work sites.
