Employee killed when struck by wing nut union from hose

Employee #1, a laborer, and three coworkers were hydrostatically testing 7 in. diameter pipe. Employee #1 was sitting on another pipe that was approximately 32 ft away from the pipe that was being tested. When the test pressure was brought up to 16,000 psi, a hammerlock union failed and parted. The female end, to which the pressure hose was attached, flew back approximately 10 ft and struck a metal backstop that had been put in front and back of the pipe being tested for the employees' protection. The part separated from the hose, ricocheted off the backstop, and struck Employee #1 on the head. He was killed.
