Employee injured in fall from job-made scaffold board

Three employees were working 22 feet above ground level from a pump jack scaffold. The scaffold board, which was job-made, broke in two near the mid-point. Employee #1 fell and fractured his hip. The scaffold board, consisting of 2 by 4s and wafer boards, was 12 feet long by 2 feet wide. Two by fours and wafer boards are not the equivalent of scaffold grade planking. The scaffold board was also overloaded for its given span.
