EMPLOYEE KILLED WHEN HE FELL THROUGH A FLOOR OPENING

AT APPROXIMATELY 8 PM ON MAY 10, 1988, EMPLOYEE #1, A BOLTER, WAS PERFORMING ROUTINE TASKS, SUCH AS CHECKING THE FLOUR MOVING EQUIPMENT, IN THE ATTIC LEVEL OF THE PACKING BUILDING OF THE NABISCO BRANDS FLOUR MILL AT 2221 FRONT STREET, TOLEDO, OHIO. HE STEPPED OR WALKED ON A 2 BY 4 FOOT FRAMED PLYWOOD COVER THAT HAD BEEN PLACED OVER A 54 INCH BY 58 INCH FLOOR OPENING. THE COVER HAD BEEN PUT IN PLACE BY AN OUTSIDE CONTRACTOR MORE THAN A WEEK BEFORE THE ACCIDENT AND HAD NOT BEEN SECURED IN PLACE. ONE CORNER OF THE COVER DROPPED INTO THE OPENING, CAUSING EMPLOYEE #1 TO FALL THROUGH. HE STRUCK A LANDING/FLOOR EDGE THAT WAS APPROXIMATELY 17 FEET DOWN WITH HIS CHEST. HE THEN FELL ANOTHER 20 FEET TO THE 1ST FLOOR, LANDING ON A GEAR REDUCTION BOX AND THE CONCRETE FLOOR. HE WAS KILLED.
