Employee killed when pinned by falling bale of plastic

Employee #1 was removing 940 lb bales of shredded plastic from a semi-trailer. One bale had been removed with a fork truck. When he went into the trailer, presumably to pull a bale from a stack of three, he apparently placed boards on the trailer floor to keep the bale elevated so that the forks could slide under it after having pulled it down. One bale fell and appears to have caught his foot while a second bale fell and struck him, pinning him on his back. Employee #1 was killed. There were no witnesses to the accident.
