Investigation Summary
EMPLOYEE #1 WAS DRIVING A CLARK ELECTRIC LIFT TRUCK (SER.NO. C62-81,0029) FROM A LOADING PLATFORM (105 3/4 INCHES LONG BY 36 INCHES WIDE) ONTO A BALCONY (11 1/2 FEET ABOVE THE FLOOR) IN BUILDING #24. HE AND THE CLARK TRUCK WERE BEING LIFTED TO THE BALCONY BY A WHITE LIFT TRUCK (SERIAL NO. 5600269). SUDDENLY THE TRUCK AND EMPLOYEE #1 FELL BACKWARDS OFF THE WHITE LIFT TRUCK TO THE FLOOR. EMPLOYEE #1 HIT THE FLOOR FIRST AND THE CLARK TRUCK LANDED ON TOP OF HIM, CAUSING FATAL INJURIES TO HIS HEAD AND ARM. THE LOAD FELL BACKWARDS BECAUSE THE WHITE LIFT TRUCK WAS NOT PROVIDED WITH AN EXTENDED BACKREST TO PREVENT LOADS FROM FALLING REARWARD.
