Investigation Summary
At 7:45 a.m. on December 11, 2014, Employee #1 was searching for a water leak in a humidifier inside of a York International PACE air handler. The employee ente red fan room #4 on the ninth floor with two other workers, one of which was a su pervisor. The employee entered the air handling room while the air blower was i n operation to search for a water leak from a drain. Employee #1 walked near th e unguarded 32 inch diameter air blower cage that housed a 42,000 cfm axial fan and was pulled into the fan while it was in operation. The employee was dismembe red and killed.
