Investigation Summary
TWO EMPLOYEES WERE TRYING TO DETERMINE WHAT WAS WRONG WITH A SCREW CONVEYOR FOR A GRAIN DRYER. THE CONVEYOR WAS RUNNING IN REVERSE. THE MAIN SWITCH ON THE CONTROL BOX WAS IN THE OFF POSITION, AND ONE OF THE EMPLOYEES OPENED ITS DOOR. THE OTHER EMPLOYEE APPARENTLY THOUGHT THE POWER WAS ON AND WAS TRYING TO SHUT OFF THE POWER MANUALLY. HIS RIGHT HAND CONTACTED EXPOSED LIVE PARTS OF THE "SERVICE-ENTRANCE CABLE" WHILE HIS OTHER HAND AND CHEST WERE IN CONTACT WITH THE GROUNDED BOX. THE EMPLOYEE WAS ELECTROCUTED.
