EMPLOYEES LOSES TIP OF RIGHT INDEX FINGER-PUNCH PRESS

At approximately 8:15 a.m., on August 11, 1988, Employee #1 lost the tip of his right index finger while using a three stage punch press. He was bending eye bolts on the press. He was placing an eyebolt in second stage. The eyebolt slipped and his right index finger slipped under a press causing the tip to be amputated.
