At approximately 11:25 a.m. on January 2, 1990, Employee #1 was laying cedar shake on a 6:12 pitched roof of a single family residential construction project. He fell approximately 20 feet from the roof. Only chicken boards (horizontal 2 by 4s) were used for fall protection.
