EMPLOYEE STRUCK AND KILLED BY FALLING TREE

EMPLOYEE #1 WAS WALKING IN A PARKING LOT NEXT TO A FRESHLY PLANTED "SABLE" PALM TREE. THE SOIL AROUND THE TREE HAD BEEN COMPACTED, BUT THE TREE HAD NOT BEEN GUYED OR BRACED IN ANY WAY. WHEN EMPLOYEE #1 WAS APPROXIMATELY 14 FEET FROM THE BASE OF THE 25 FOOT TALL TREE, IT UPROOTED AND FELL ON HIM, STRIKING HIM ON THE HEAD, AND LANDING ACROSS HIS BODY. THE EMPLOYEE WAS KILLED. NO VIOLATIONS OF INDUSTRY PRACTICES OR PROCEDURES WERE APPARENT.
