EMPLOYEE #1 WAS CLEANING PAINT SPRAY EQUIPMENT WITH TOLUENE IN A PAINT MIXING ROOM. THE EMPLOYEE WAS FOUND FACE-DOWN IN A 3 FOOT BY 3 FOOT TANK OF TOLUENE. EFFORTS TO REVIVE THE EMPLOYEE WERE FUTILE.
