EMPLOYEE CAUGHT BETWEEN TRACTOR AND TRAILER AND CRUSHED
Employee #1 was inscribing inspection data on the front of a new trailer when the truck-tractor assigned to pick up the trailer backed into him. Employee #1 was pushed under the trailer and lodged between the tractor's fifth wheel and the hitching attachment of the trailer. Employee #1 was crushed to death.
