Investigation Summary

Investigation Nr: 571240
Event: 07/03/1988
Employee killed in fall through skylight

Employee #1 was alone in a plant washing trucks on a Sunday. He went on the built-up tar and gravel roof of the plant to watch fireworks and sat on the edge of an unguarded skylight. In front of the skylight were later found two heel marks, the backs of which were deeper than the front, indicating that he was sitting or possibly leaning back on the skylight when it gave way and he fell approximately 20 feet to a concrete floor. Employee #1 died of a concussion and a fractured skull.
