Employee dies after falling from roof

Employee #1 and his brother were assigned to perform tasks on a roof 28 ft above ground level. They were working approximately 18 in. from the edge of the roof. Employee #1 apparently fell off the roof without his brother being aware that he had fallen. Employee #1 died ten days later in the hospital of complications resulting from injuries sustained in the fall. No mechanical equipment was being used on the roof at the time the accident occurred. No safety monitoring system was in use.
