EMPLOYEE KILLED IN FALL FROM ROOF

An employee was working on a roof about 25 ft above the ground. He was not using fall protection nor was a catch platform installed. He either slipped or stumbled and slid off the roof. The employee fell to the ground, sustaining a ruptured diaphragm. This injury allowed the contents of the abdominal area to enter the thoracic cavity, causing the employee to suffocate.
