CONSTRUCTION WORKER KILLED IN FALL DOWN ELEVATOR SHAFT

AT 10:10 AM ON OCTOBER 7, 1985, EMPLOYEE #1 FELL DOWN AN ELEVATOR SHAFT LOCATED IN A BUILDING UNDER CONSTRUCTION. EMPLOYEE #1, AN IRONWORKER, WAS STANDING ON SOME TEMPORARY DECKING ASSISTING ANOTHER IRONWORKER WHO WAS WORKING ON A FLOAT SCAFFOLD IN THE ELEVATOR SHAFT. THE OPENING AROUND THE SHAFT WAS PROVIDED WITH A SAFETY RAILING AS PER 1926.750 (B)(1)(III). AS EMPLOYEE #1 LEANED AGAINST THE RAILING, THE ANCHORAGE FOR THE SAFETY LINE, A WASHER WELDED TO A COLUMN, GAVE WAY. EMPLOYEE #1 FELL 50 FEET TO THE CONCRETE FLOOR OF THE ELEVATOR PIT. CITATIONS WERE RECOMMENDED FOR IMPROPER ANCHORING OF THE LINE AND NOT PROVIDING SAFETY NETS WHEN THE POTENTIAL FALL DISTANCE EXCEEDED 25 FEET.
