EMPLOYEE #1 WAS STRAIGHTENING LUMBER IN A BOXCAR. THE MICRO-LOCK BRAKE OF A FORKLIFT FAILED, ALLOWING IT TO ROLL INTO EMPLOYEE #1. THE EMPLOYEE WAS KILLED WHEN HE WAS CRUSHED BETWEEN THE FORKLIFT AND THE BOXCAR.
