CONSTRUCTION EMPLOYEE KILLED WHEN LADDER SLIPPED AND FELL

EMPLOYEE #1 WAS WORKING WITH A CREW OF 4 MEN SEALING 2 BY 4'S WITH ROOFING CEMENT ATOP A BUILDING ROOF. EMPLOYEE #1 BEGAN DESCENDING A METAL LADDER. THE LADDER SLIPPED, CAUSING EMPLOYEE #1 AND THE LADDER TO FALL 12 FEET TO A CONCRETE FLOOR. HE WAS KILLED.
