CONSTRUCTION EMPLOYEE KILLED IN FALL FROM ROOF

ON AUGUST 19, 1987, AT APPROXIMATELY 2:05 PM, EMPLOYEE #1 WENT ONTO THE ROOF OF A BUILDING UNDER CONSTRUCTION. A FORKLIFT TRUCK LIFTED A PURLIN WITH 9 PANELS INTO PLACE. EMPLOYEE #1 WENT TO THE SOUTH SIDE OF THE PURLIN AND NAILED IT INTO PLACE. WHILE PROCEEDING TO THE NORTH SIDE, EMPLOYEE #1 NAILED THE PANELS TO THE EXISTING ROOF STRUCTURE. WHEN EMPLOYEE #1 ARRIVED AT THE NORTH SIDE AND PREPARED TO NAIL THE PURLIN JOIST HANGER, IT BROKE. THE PURLIN, PANELS, AND EMPLOYEE #1 FELL TO THE CONCRETE FLOOR. HE WAS KILLED. EMPLOYEE #1 WAS NOT PROTECTED BY ANY TYPE OF FALL PROTECTION.
