RUN OVER BY NUT HARVESTING MACHINE

EMPLOYEE #1 WAS KILLED WHEN HE FELL FROM THE SEAT OF A FLORY 6500 NUT HARVESTING MACHINE, SERIAL NUMBER A H002. HE LEANED OUT FROM THE DRIVER'S STATION TO CHECK WHETHER ALL THE NUTS WERE BEING PICKED UP, LOST HIS BALANCE AND FELL TO THE GROUND. HE WAS RUN OVER BY THE HARVESTER'S LEFT REAR WHEEL AND BY THE TRAILER BEING TOWED BY THE HARVESTER. THE HARVESTER WHEEL PASSED OVER HIS UPPER TORSO AND THE TRAILER (3/4 FULL OF NUTS) ROLLED OVER HIS LOWER TORSO. HE WAS CRUSHED TO DEATH.
