Investigation Summary
Employee #1 was crushed to death when a grain auger fell onto him. The Model 4100 auger was being modified so as to extend an additional 10 feet further into the grain bin. Employee #1 was lifting the auger off the ground by using a winching mechanism which was attached to the truck that carried the auger. At one point he was unable to winch it higher. It was approximately five feet high, which was lower than he wanted to go. He went to see what was keeping it from going higher. In doing so, he positioned himself directly underneath the auger. One of the bolts which held the pulley mechanism came out causing the auger to come down, striking and killing him.
