Employee crushed and killed between truck dump box and frame

Employee #1 was working by himself repairing a truck frame on a 1974 Ford 24 ton dump truck, PSC-5527, with a 14 foot box of 10 yard capacity, and 1970 Heil hoist. A trip wire ran the length of the frame to the end of the box to prevent the box from being raised too high. A manually-operated support arm was available to prevent accidental lowering of the box during repair work. Employee #1 was positioned above the last axle with his head on the passenger's side and his legs resting on the wheels of the driver's side. Apparently, he accidentally pulled the trip wire, causing the box to lower and pin him to the frame. Employee #1 died of a crushed head and lower torso. He was discovered around 3 p.m. by a worker from the prime contractor who was passing by.
