EMPLOYEE #1 WAS WORKING IN A 15 FEET DEEP TRENCH IN A LANDFILL THAT WAS NOT SLOPED, SHORED OR OTHERWISE PROTECTED FROM CAVE-INS. ONE OF THE BANKS COLLAPSED ON EMPLOYEE #1, KILLING HIM. AN EMPLOYEE FOR ANOTHER CONTRACTOR WAS ALSO KILLED IN THIS ACCIDENT.
